How to Apply

The senior/disabled waiting list is currently open for application. In order to apply, you will have to fill out an application and submit the following documents along with the application: Birth certificates for each family member, social security cards for each family member, if family members were not born in the United States, verification of citizenship or permanent residency (Green Card), verification of current earnings of all family members, i.e. current pay stubs (4 if paid weekly, 2 if paid bi-weekly) or a letter from employer(s), award letter(s) showing the amount of Social Security benefits, public assistance, pension unemployment benefits, copies of current bank statements or other proof of assets i.e. Certificates of Deposit, credit union savings, stocks/bonds, dividends, propwrty owned. In order to recieve an application please pick one up at the office located on 249 Milbank Avenue, Greenwich, CT 06830 or call us at 203-869-1138 to request it by mail.
